Nurse Staffing Information Available under the Illinois Hospital Report Card Act

The mission of St. Mary’s Good Samaritan, Inc. (St. Mary’s Hospital in Centralia, IL and Good Samaritan Regional Health Center in Mt. Vernon, IL) is to provide the very best care for our patients.  The methods used to schedule and assign nurse caregivers to patients are consistent with this mission.  Nurses at St. Mary’s Good Samaritan, Inc. complete an orientation to the facility as well as the area in which they work.  The caregiver assignments are reviewed and changes made based upon patient needs.

The Illinois Hospital Report Card Act (“Act”) describes the information that patients, family members and the public may request, including the following nurse staffing information:

  • Nurse assignment rosters (Individually identifiable patient, employee or licensed professional information will not be disclosed.)
  • Nursing schedules for each inpatient unit (Individually identifiable patient, employee or licensed professional information will not be disclosed.)
  • Methodology used to determine and adjust nurse staffing levels
  • Information on staff training and orientation 

Individual patients or their family members may contact the nurse in charge of the patient’s unit for information about nurse assignments. 

Other persons requesting information under the Act are required to complete a Request for Information Hospital Report Card Act data request form before the information is disclosed.

Once the form has been signed and submitted, St. Mary’s Good Samaritan, Inc. will then provide the appropriate information either verbally or in a format that may be viewed by the requestor.
